I think that pen sizes bring smaller back in the day has more to do with the pen grip having changed. Look at the Palmer/Spencerian grip. It's more like a dip pen grip where you basically only have your index finger on the section, on the top side of the pen, and use it for flourishes and the hand position has the back of your have facing upward instead of off to the side.
